Can you guess the most common skin condition in the US? If you guessed acne, you got it! Almost everyone has or knows someone who has suffered from acne. Not all acne is the same either, there are many different types so I want to just take a bit to go over those.
Most people know what acne is, which is characterized by pimples. Acne is also any whiteheads, blackheads, nodules, or cysts and can affect people on their face, neck, shoulders, chest, back, upper arms, and upper legs. Acne doesn’t discriminate and is not confined to any race, age, or gender. Let’s go over the different types:
Comedones: A comedo (singular) is a basic acne lesion and is caused when the hair follicle becomes clogged with oil and dead skin cells. There are two different types of comedones: blackheads and whiteheads. Blackheads are when sebum becomes blocked and cannot drain and bacteria starts to build up. They are not black from dirt, but from the melanin that is in your sebum that oxidizes when exposed to the air. A whitehead is different in that it is a closed pore. Oil and skin cells prevent the clogged pore from opening and so it swells underneath the skin. It’s hard to not want to pick and squeeze these two but doing so in unclean conditions will just cause further infection. It’s better to not pick.
Papules: These are inflamed comedones, and they form small red/pink bumps on the skin. Papules contain no pus or fluids, they’re usually less than 1cm in diameter, and may even be sensitive to the touch. It may be hard but try not to pick or squeeze as it can make the inflammation worse or even lead to scarring.
Pustules: Small and inflamed pimples filled with pus and contain a red ring around it. It looks similar to awhitehead with red around it. A pustule has white/yellow pus which is, usually, a mixture of bacteria, white blood cells, and dead skin cells. They, typically, form around a hair follicle or sebaceous gland. Again, don’t pick because it will lead to scarring or dark spots on your skin.
Nodules: One of the most painful types of acne, it is a solid lesion that is large and inflamed, and solid to the touch. They’re painful because the extend to the deeper layers of your skin and can be known to even do tissue damage. These are harder and more painful to remove, doing so yourself can lead to scarring.
Cysts: They are also lesions that contain pus like in pustules. Cysts are larger and much more infected than pustules, though. Nodules and cysts are both painful and considered to be severe forms of acne. Cysts also extend to the deeper layers of your skin and leave behind scar tissue.
Nodulocystic Acne: It is a severe form of acne that can be on the face, chest, shoulders, and back. It’s characterized by having multiple inflamed cysts and nodules and is painful. Typically, it’s a deep red or purple acne and because it goes deep into the layers of your skin it often leaves scarring. This is more often found in men than women.
Acne Congoblata: One of the most severe forms of acne because it involves many inflamed nodules that are connected under the skin to other nodules. There are groups of large macrocomedones and cysts filled with smelly pus. Mostly on the neck, chest, arms and buttocks. This acne results in heavy scarring and is more often found in men. Especially if using steroids or testosterone.
Acne Mechanica: Known as the “sports-induced acne” because it occurs in athletes frequently. This acne is caused by heat, friction, and pressure against the skin. Usually, from wearing sport gear like helmets and caps. It can vary from small comedones to large pustules or papules.
Mild Acne: You’re considered to have just mild acne if you have around less than 20 blackheads/whiteheads, fewer than 15 inflamed bumps, and fewer than 30 total lesions. Mild acne is more common and easier to treat as well.
Moderate Acne: This is defined by having between 20-100 whiteheads/blackheads, 15-50 inflamed bumps, or 30-125 total lesions. For moderate acne it, typically, seems to get worse before it gets better.
